45M neckpain from a week

Patient came to the OPD with the chief complaints of neck pain from a 1week
HOPI:
 neckpain was insidious in onset and gradually progressive  which is of dragging type
No history of headache, tingling sensation, tremors
Past history nil 
N/o diabetes mellitus, hypertension, asthma, T.B,CVA, CAD, thyroid
personal history:
appetite  is  normal 
mixed  diet
sleep is adequate 
bowel and bladder movements are normal 
occasional alcoholic 
and no other  addictions 
his pulse was :64bpm
resp.rate:18rpm
b.p:90/60mmof hg
