Patient came to the OPD with the chief complaints of neck pain from a 1week HOPI:  neckpain was insidious in onset and gradually progressive  which is of dragging type No history of headache, tingling sensation, tremors Past history nil  N/o diabetes mellitus, hypertension, asthma, T.B,CVA, CAD, thyroid personal history: appetite  is  normal  mixed  diet sleep is adequate  bowel and bladder movements are normal  occasional alcoholic  and no other  addictions  his pulse was :64bpm resp.rate:18rpm b.p:90/60mmof hg

Patient came to the OPD with the chief complaints of Fever cold cough back pain since four days Fever was sudden and onset and gradually progressive  fever increases during night and decreases during morning  backpain was dragging type and aggrevates on fever  dry cough no significant past history N/o of diabetes mellitus, asthma, hypertension, thyroid, CAD, CVA, T.B personal  history  : appetite is  reduced mixed diet sleep is adequate bowel and bladder movements  are normal  no addictions   general examination  : no pallor  no icterus  no cyanosis  no clubbing  no lymphadenopathy  no edema  her pulse was 72 bpm  resp.rate 16rpm b.p 120/80mm of hg

patient came to  the OPD with the cheap compliants of  cold Since 4 days and fever, body pains, cough, neck pain since 3 days Fever is Insidious in onset and gradually progressive leg pains are of throbbing type from mid thigh to ankle and radiates to t h i g h during sleeping  retro orbital brain is present  burning sensation in Eyes is present from 3 days  no significant part history  no history of diabetes mellitus hypertension asthma TB CVA  CAD   personal history  sleep is disturbed appetite is reduced  mixed diet  burning micturition since 3 days bowel movementsare normal occasional alcoholic drinker  no other addictions  no significant family history  general e examination  no pallor no icterus  no lymphadenopathy  no edema no cyanosis no clubbing  his pulse was 76 BPM  respiratory rate 26 bpm  blood pressure 110 / 70 mm of hg

 45F came to the opd with c/o white discharge from2-3 months and fever in nights  and body pains from 2months  and loss of weight in past 10 days about 3kgs  white discharge is like watery  consistency  fever is sudden and onset and relieves on medication  body pains are of dragging type  no significant past history  N/o asthma, thyroid, diabetes mellitus, CAD, CVA  she is a known case of hypertension  since 3years and on medication  of amlodipine 5mg  personal history: appetite is  normal  diet is mixed  sleep is disturbed  due to body pains  bowel and bladder movements  are normal  no addictions  general examination  : no pallor   no icterus  no edema  no lympahadenopathy  no cyanosis  no clubbing  pulse :66bpm b.p :160/100mmof Hg resp.rate :29rpm
